What is a Spanish Health Control Form?

The government of Spain has followed in suit, the other western countries, and made it compulsory for all those who intend and plan to travel to Spain amidst the COVID-19 pandemic, to fill the Health Control Form. Travelers are required to fill the form within 48 hours of the intended trip. It is the same form which is commonly termed as the Passenger Locator Form in other western countries.

WHO ALL NEED PASSENGER LOCATOR (HEALTH CONTROL) FORM?

As per the Royal Decree-Law 8/2021 of 4th May, all passengers entering Spain from any seaport or airport located out of Spain, are to fill the Health Control Form. Visa required travelers will need to have their approved visa as well. For a lot of travelers, a Schengen visa will suffice, according to natvisa.com. Those international passengers who only have to transit at any airport of Spain before taking off to other countries are also required to fill the form as per the law, QR for transit is known as TRANSIT CONTROL QR.

WHY PROVIDE PERSONAL DETAILS?

All the details asked in the form are to ensure the health safety of the travelers as well as the citizens of Spain. It is obtained in pre-emption of the risk if you are identified symptomatic or found in close contact of any positive COVID case, to avoid the spread of health hazards. Your email address will be your point of contact during your stay in Spain. In case you do not have a valid email ID, you may add the email address of the family member or a member of the group which is traveling together.

WHAT INFORMATION DO YOU NEED TO PROVIDE?

  • While filling the form, you will be asked the following details: -
  • Your valid passport and the National Identity Card.
  • Details regarding your travel plans including date and time of arrival, flight details, seat numbers, etc.
  • Your accommodation plan during your stay in Spain.
  • Email address for confirmation of QR Code and any contact.
  • Vaccination certification, PCR Test documents, and negative reports for countries in high-risk zones of the world. You can check for your country from the RISK COUNTRIES/AREAS link.
  • Children (less than 12 years) may not be asked to provide above mentioned documents.
  • In case you do not know the exact place where you intend to live, you can only provide the city name and the community area where you may go. Just with this much info, the QR Code will be sent to your email.

PROCEDURE TO PROVIDE YOUR DOCUMENTS IN THE FORM?

  • EU country residents can easily upload the digital EU COVID certificate on the SpTH (Spain Travel Health) application by attaching the certificate in the Health Questionnaire, section 4.
  • If everything is found correct, a QR will be forwarded to you. Paper / printed certificates can also be added in the form by selecting Paper format in the same section.
  • In case you do not possess the EU digital COVID certificate, even then you can get the QR code.
  • You just have to enter the correct details of the supporting documents related to your vaccination and the COVID – 19 Test in section 4 of the Health Questionnaire.

WHAT IS THE QR CODE AND HOW TO PRESENT IT AT THE AIRPORT?

Quick Response code is received when you correctly fill the Health Control Form (Passenger Locator Form). This QR code is to be provided to the airport authorities at the entry airport of Spain. Most likely, you will be requested to display and scan the issued QR Code through designated devices at the terminal. It can be provided in a printed form or you can simply provide it on an electronic device (mobile or tablet) using the SpTH application. Children under 12 years of age will only be required to fill the FCS to obtain the individual QR code.

HOW TO RECEIVE A QR CODE FOR A FAMILY OR GROUP?

QR code can also be obtained for a family or a group but for that representative must fill in the data of all the family members by selecting the correct options on the Health Control Form.  In the NEW FORM option, you can enter the FAMILY / GROUP tab, followed by the FCS form button to add details of family members or a group. It is advised to fill in all the information correctly to avoid any inconvenience at the airport on arrival in Spain. For the ease of passengers, it is suggested to provide an email of all the members so as each member receives a personal QR code.

CAN THE DATA BE CHANGED AFTER RECEIVING THE QR CODE?

Some parts of the information provided in the form can be changed but within the provided time limits. On making the modifications, a new QR Code will be sent to the provided email address. When a change is intended to be made, you will provide a passport or national identity number to sign in and a code will be sent to the email ID to proceed with the changes.

MISCELLANEOUS INFORMATION

  • QR Code can be retrieved in case it is lost, by going into the RETRIEVE QR CODE option in the SpTH application or the home page on site.
  • If a member of a group or family is unable to travel, do not worry, his QR code will expire automatically after 7 days of the expected arrival date.
  • In case you suffer any symptoms during your stay, you must approach the health authorities for your safety and the safety of your family and travel group.
